The Handoff Failure
During the rough-in phase of a commercial mid-rise project, the mechanical team encountered a structural steel clash that prevented the installation of the main waste line according to the shop drawings. To keep the project on track, the plumbing subcontractor modified the pipe routing in the field, bypassing the obstruction by lowering the run by eighteen inches. The field team completed this adjustment without formalizing the change through a revised layout drawing or updating the shared coordination model. They assumed the slight deviation would not impact other trades.
Two days later, the electrical subcontractor began installing heavy conduit runs along the designated overhead corridor. Following the coordinated model, they placed the conduit racks exactly where the lower waste pipe now resided. This physical overlap went unnoticed until the crews physically met in the corridor, resulting in a complete halt of both installations. Neither trade had space to route their systems without violating code clearances.
