A Closed Item Still Had an Open Question

This failure highlights a classic case of information masking where software metadata hides actual engineering reality. The system recorded a binary state (Closed) which led the project team to stop tracking the item. The administrative workflow lacked a verification gate to confirm that conditional approvals were communicated to the field.

In this instance, three key breakdowns occurred. First, the closure of the RFI was treated as an administrative milestone rather than a technical handoff. Second, the system did not automatically flag the conditional approval as an open task or field constraint. Third, there was no direct handoff protocol between the coordinator who closed the RFI and the superintendent managing the mechanical installation. As a result, the critical instruction was lost in the digital archive, while the physical construction proceeded based on outdated assumptions.

To prevent information masking from stalling future projects, we established a strict protocol for closing items with outstanding actions. First, no RFI or checklist item with conditional parameters may be marked "Closed" until the conditions themselves are logged as independent, tracked tasks in Procore.

Second, we instituted a weekly handoff review meeting specifically focusing on recently closed items to ensure that any field-level dependencies are transferred to the superintendent's daily plans. Finally, we recommend training administrative staff to never close an item based solely on the engineer's signature without reading the response text for hidden contingencies. Implementing these steps ensures that a closed status in the system truly reflects a resolved condition in the field, eliminating downstream rework and costly delays.
